FTR Recording window shuts down

I am attaching the error messages we are getting when the Log Note window shuts down on the recording PC .  We will be in the middle of a hearing and the window will shut down.   The recording is still capturing the audio but the log note windows just closes.  

Any help would be greatly appreciated
